*Please be advised that all programs require a two-month commitment. Once scheduled, attendance is mandatory for your online session to be eligible for payment with your health savings account (HSA). No refunds will be issued for missed prescription appointments.

*Payment of all fees is expected at the time of service. All weight-loss prescriptions are monthly subscriptions that are only eligible for cancellation after two months. All payments are non-refundable.
